We utilize holistic health services for the services we offer. With this, we can understand our patients’ health as a whole and consider the different facets of their health in creating a care plan that fits their needs and preferences. Each client, even those with similar conditions, has needs and preferences that are unique for them. As primary care providers, we prioritize empathy, professionalism, and respect. Our health care professionals are all licensed and specialize in different areas of care to offer a wide variety of services. We currently offer the following:

Annual Physical Exam

Annual Physical Exams involve a series of routine examinations that a patient undergoes once a year. The exam results can help you and your doctor assess the different areas of your health and take a closer look at areas that need further looking into. These sets of exams also help detect the early signs of health issues that, if detected early, can be treated earlier. Patients may also update their vaccinations during their annual physical exams.

Chronic Care Management

Chronic health management aims to help patients maintain a good quality of life by offering continuous health monitoring and management. The services patients receive under this program depend on their condition and the level of support they need. They may receive evaluation and advice regarding their diet, prescribed medications, exercise and physical activities, and more.

Acute Illness Management

Acute illness management is utilized to evaluate and treat acute illnesses (illnesses that may develop suddenly and rapidly and generally have a shorter span). This form of health management is usually provided for conditions that result from injuries, over or underdosing of substances or medications, sudden body pains, burns, diarrhea, and cough and cold.

Urgent Care
Health concerns that need immediate attention but are not considered emergencies or require complex and specialized medical procedures can be treated with urgent care. At Unified Primary Care and Health Services LLC, our team of health professionals can treat different urgent care conditions; including falls and accidents, fever or flu, eye or infection, sore throat and bronchitis, skin rashes, UTI, vomiting, diarrhea, dehydration, falls, sprains, strains, broken bone, cuts and burns, and animal bites, among others.

Weight Management
Weight Management involves determining health factors that need to be changed to help a patient maintain a healthy weight. It may involve strategies that can help an individual gain, lose, or maintain a certain weight range based on their sex, age, and height. With weight management, patients talk to health professionals about their goals and implement dietary, physical activity, and stress management adjustments to promote a holistic wellness improvement.

Employee Health Services
Employees need to have convenient access to health care so their health can easily be monitored. Though the services covered are different per company, they usually constitute immunizations, physical examinations, and medical screening tests. Employees may also benefit from receiving services that may include first aid or emergency response, physical fitness, health education, health intervention activities, and preventive health screenings.

School, Sport, and Camp Physicals
Many schools require children to undergo physical exams before they can attend classes. This will help the school assess their development and ensure that their immunizations are updated to get protected from different diseases. On the other hand, most sports teams and some camps need their members to undergo physical tests to ensure that they are fit to participate in the activities. The results of these exams also list medications, visual aids (eyeglasses and contact lenses) that schools, sports teams, and camps need to take note of to ensure students’ safety.

Medical Marijuana Evaluation and Recommendation
Many individuals want to use medical marijuana to manage symptoms related to their health conditions. Since marijuana has shown some evidence of its calming or healing effects but can also easily be abused by users, individuals need to undergo evaluation and receive a recommendation from their doctors. A doctor’s recommendation is a requirement in applying for a permit to access and use medical marijuana.

DOT Physical Examination
DOT Physical Examination is a requirement for commercial drivers so they can be eligible to drive commercial trucks and vehicles on the road. The exams they have to undergo will test their overall fitness to prove that they can last long hours on the road and control factors like stress and fatigue. Specific tests, like eyes exams, are also included to ensure they can see the road clearly and avoid causing or getting involved in accidents.

Travel Consults and Immunization
Those planning to travel to unfamiliar places, especially outside the country, need to visit their doctors for advice about possible health risks and how to avoid or minimize them. Their health history and itinerary are assessed and be provided health and security tips, including preventive measures for dietary and recreational activities. They will also be recommended certain immunizations to protect themselves from possible infections.
